Our Story

Discharge planning is hard. The tools never kept up.

Every day across California, thousands of patients are clinically ready to leave the hospital but stuck waiting for a post-acute bed. Hospitals, skilled nursing facilities, assisted living, board & care homes, memory care, and hospices each hold one piece of the picture — but there has never been a shared, real-time view of which beds are open and who needs them.

So discharge planners phone and fax dozens of facilities for a single patient. Facilities field referrals that are incomplete, mistimed, or a poor clinical fit. The work isn't broken and the people aren't failing — everyone is doing their best with the information they can get. The tools simply never kept up with the urgency of the moment.

BedGrid Care Network, Inc. is a California nonprofit public benefit corporation. We are organized exclusively for charitable and educational purposes — to improve care transitions across the post-acute continuum — and we do not exist for the private gain of any person.

Being a nonprofit isn't a marketing choice; it's a structural commitment. It is why BedGrid is, and will remain, free for every hospital, every post-acute provider, and every patient and family who uses it. No subscription tiers. No commissions. No paywalled features. Ever.
